When a physician bills E/M visit and Preventative Screening would modifier 25 be appended to both codes or just the E/M? Example would be 99213-25, G0042
It is my opinion that the modifier goes on the E/M only but a co-worker said it goes on both. I can't find supporting documentation for either scenario.
