The claim denied for Medical Unlikely Edits. The number of units is 88
11200 @ 1 unit, and 11201 @ 7 units
11200 @ 1 unit and 11201 @ 7 units 51 modifier
11200 @ 5 units and 11201 @ 1 unit
11200 @ 5 units and 11201 @ 1 unit 51 modifier

My vote is 11200 @1 unit and 11201 @ 7 units 51 modifier.

Looking at the description of Modifier -51 in Appendix A, you will see this note: "This modifier should not be appended to designated "add on" codes " therefore, we don't use -51 with the add-on code of 11201.

You stated there were 88 skin tags.

11200 X 1 will document the first 15 skin tags. That leaves 73 more skin tags. The code 11201 states "Removal of skin tags... each additional 10 lesions, or part thereof." This means you will actually need 11201 X 7 to code the next 70. Then you need one more 11201 to code the additional "part thereof", namely the 3 remaining skin tags.

None of the answers you suggested are correct. The correct codes would be:
11200, 11201 X 8
